Recommendations for Practice: Prevention & Treatment of Pressure
Ulcers
- Complete patient history and physical examination to determine general
health and risk factors that may delay healing.
- Assess and modify situations where pressure may be increased (e.g.,
when seated or lying down).
- Assess and control pain.
- Maximize nutritional status.
- Control moisture and incontinence.
- Maximize activity and mobility, reducing or eliminating friction and
shear.
- Assess and assist with psychosocial needs and develop a patient-centred
plan.
- Stage, assess, and treat the wound to provide an optimal wound environment
(debridement, infection control, moisture balance, biologicals).
- Introduce adjunctive modalities if clinically indicated.
- Consider surgical intervention for deep nonhealing ulcers (Stage III
and IV).
- Develop an interdisciplinary team with flexibility to meet the patients
needs.
- Educate patient, caregiver, and health care professional on the prevention
and treatment of pressure ulcers.
